Paroxetine HCl
Indications/Uses
Major depressive disorder (MDD), panic disorder w/ or w/o agoraphobia, premenstrual dysphoric disorder (PMDD), social anxiety disorder/social phobia in adults.
Dosage/Direction for Use
Adult ≥18 yr MDD Initially 25 mg daily, may be increased by 12.5-mg daily increments up to max: 62.5 mg daily. Panic disorder Initially 12.5 mg daily, may be increased by 12.5-mg daily increments up to max: 75 mg daily. PMDD Initially 12.5 mg daily, may be increased to 25 mg daily. Social anxiety disorder/social phobia Initially 12.5 mg daily, may be increased by 12.5-mg daily increments up to max: 37.5 mg daily. Elderly Initially 12.5 mg daily, may be increased up to 50 mg daily.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Not to be used w/ MAOIs, thioridazine & pimozide.
Special Precautions
Discontinue use if serotonin syndrome or NMS-like events, seizure occurs. Monitor for worsening &/or emergence of suicidal ideation/behaviour or thoughts. Akathisia; history of mania; bipolar disorders; bone fractures; hyponatraemia. Cardiac conditions, epilepsy, narrow-angle glaucoma, known tendency for bleeding or w/ predisposing conditions. Avoid abrupt discontinuation. Not to be used w/ serotonin-precursors eg, L-tryptophan, oxitriptan. Concomitant use w/ tamoxifen, MAOIs, ECT. May affect ability to drive & use machines. Hepatic & severe renal impairment. Pregnancy. Not to be used during lactation. Not for use in childn & adolescents <18 yr. Elderly. 12.5 mg: May cause allergic-type reactions due to azo dye colouring agent.
Adverse Reactions
Nausea; sexual dysfunction. Increased cholesterol levels, decreased appetite; somnolence, insomnia, agitation, abnormal dreams; dizziness, tremor, headache; blurred vision; yawning; constipation, diarrhoea, vomiting, dry mouth; sweating; asthenia, wt gain; sensory & sleep disturbances, anxiety.
Drug Interactions
Serotonin syndrome w/ L-tryptophan, triptans, tramadol, SSRIs, lithium, fentanyl, St. John's wort. Increased pimozide levels. Decreased plasma levels w/ fosamprenavir/ritonavir. Increased plasma levels of procyclidine. Prolonged neuromuscular-blocking action of mivacurium & suxamethonium. Increased plasma conc of TCAs, phenothiazine neuroleptics, risperidone, atomoxetine, type 1C antiarrhythmics, metoprolol. Reduced plasma conc of endoxifen. Not recommended w/ alcohol.
